Process Self-Assessment Tool
(CHIPSAT) : where are we at with

There are 3 components of Community Health Improvement that each Health Council in
New Mexico utilizes. These are "community building", "community assessment and prioritization",
and "community action". Each year, the Health Councils across the state are asked to assess
how they are doing in these areas. This document is a summary of this self assessment.
